In 2015, we joined ASPA Connected Community, an ACO based in Phoenix, Arizona. The ACO was formed by the Arizona State Physicians Association (ASPA). CMS contracted with the ACO under the Medicare Shared Savings Program to manage and provide quality care for Medicare Part B patients in Phoenix, AZ and Las Cruces, NM.
